Course Description

This learning module will introduce primary care providers (PCPs) to the evolvedMD enhanced psychiatric collaborative care model (CoCM). Through completing these modules, PCPs will understand the framework of CoCM and their role within the model. PCPs will also gain enhanced knowledge and skill in completing evidence-based screening and intervention. Ultimately, this module serves to engage PCPs in providing collaborative patient-centered care.
